A tibial pilon fracture is a high-energy break at the bottom of the shinbone (tibia) that shatters the weight-
bearing surface of the ankle joint. Because it damages both bone and cartilage, it’s one of the more complex
lower-leg injuries and requires specialized surgical care to restore the joint and protect long-term function.
Open Reduction and Internal Fixation (ORIF) uses custom-contoured plates and screws to rebuild and stabilize
the shattered joint surface as precisely as possible. The goal is to restore the ankle’s normal anatomy, because
an accurately reconstructed joint surface is the single biggest factor in reducing long-term pain and arthritis.
The plates and screws are designed to stay in place permanently and usually cause no problems. In some cases,
if the hardware causes irritation after the bone has fully healed, it can be removed in a minor follow-up
procedure. Most patients never need them taken out.
Yes. We use CT scans to create a detailed 3D map of the bone fragments, which is essential for precise surgical
planning. A standard X-ray alone can’t reveal the full complexity of a pilon fracture. The CT guides exactly how
we reconstruct the joint surface for the best possible alignment.
Most patients return to an active lifestyle after a tibial pilon fracture, though high-impact sports typically
require 6–12 months of intensive physical therapy and a gradual return. Recovery varies by the severity of the
injury and how the joint heals. Your surgeon and physical therapist will guide your safe return to activity.
Because pilon fractures cause severe soft-tissue swelling, surgery is often performed in two stages. First, we
stabilize the ankle with an external frame to protect the swollen tissue (Stage 1). Once the swelling subsides,
usually 1–2 weeks later, we perform the permanent internal fixation (Stage 2). This staged approach reduces
complication risk.
Patients typically cannot put weight on the ankle for 6–12 weeks to ensure the joint surface heals correctly.
Bearing weight too early can disrupt the reconstruction. Your surgeon will guide you through a carefully staged
return to weight-bearing, supported by physical therapy, to protect the repair and rebuild strength safely.
Unlike a simple fracture, a pilon fracture crushes both the bone and the cartilage of the weight-bearing joint
surface. If the joint isn’t perfectly realigned, it can lead to permanent stiffness, arthritis, and chronic pain. This
is why specialized surgical reconstruction is so important for a good long-term outcome.
There is a real risk of post-traumatic arthritis with any pilon fracture because of the joint-surface damage.
However, our meticulous reconstruction techniques are specifically designed to minimize that risk by restoring
the joint as accurately as possible. Ongoing follow-up helps us manage your joint health long after the fracture
